The following form and its associated guidance have the status that the Local Authority and PCT are committed to use them as they are compatible with the Local Authority Single Assessment Process forms.
This form is also a useful tool for making sure that decision-specific capacity assessment is being properly carried out and provides evidence that this has happened.
If you are daunted at the thought of yet another form, it’s worth remembering that this only needs completing for ‘big’ decisions, such as a proposed long-term accommodation move or serious medical treatment.
